Community to Home Program

The Community to Home (C2H) program improves the health of children and youth with special health care needs (CYSHCN) by assisting them, along with their families, to access the services and supports required to thrive in the community and develop to their full potential.  C2H assists CYSHCN and their families to navigate systems and identify resources in order to receive services while empowering them to become strong advocates and self-reliant.  This program utilizes evidence based, home visiting services through the Community Health Worker (CHW) model to provide care-coordination and education allowing CYSHCN and their families to learn the skills they need to succeed in living with their special health care needs.

C2H provides services within 48 rural counties in Pennsylvania through collaboration between the PA Department of Health and two grantees, CareStar and Health Promotion Council. View the counties where C2H services are available and which grantee serves your county.

In order to be eligible for the C2H Program an individual must meet the following eligibility requirements:

  • Children 21 years of age and under
  • Newly diagnosed children with special health care needs OR at risk of a diagnosis OR has previously been diagnosed but has never received services and supports OR newly residing in a PA rural county and has a diagnosis
    • "Newly diagnosed" is defined as the initial identification and determination of a physical, emotional, behavioral, or developmental need by a professional within the past year.
  • Households with an income of 300% or less of the federal poverty level
