In rural Pennsylvania, hospitals, clinics, and health care workers face unique challenges—but also great opportunities. The Commonwealth is working with local communities, providers, doctors, nurses, and leaders to improve rural health care, keep hospitals strong, and make care more affordable.

Geisinger Jersey Shore Hospital $     1,000,000.00Program payments will be used to support this technology and infrastructure project focused on expanding availability of advanced diagnostic imaging through the deployment of a portable Computed Tomography (CT) unit at Geisinger Jersey Shore Hospital (GJSH) which serves rural and underserved communities across the region. Portable CT scanners can be utilized in various locations within a hospital, including bedside imaging in the
Intensive Care Unit (ICU), operating rooms, emergency departments, and specialty care suites.

Geisinger Lewistown Hospital $     1,000,000.00Funding will support the purchase, deployment, and installation of a CT imaging unit to ensure consistent, local access to advanced diagnostic imaging services. This investment  will allow the community to maintain uninterrupted access to CT services, reducing reliance on external facilities and minimizing delays caused by equipment outages. Central
Geisinger Medical Center $     1,000,000.00This project is an access and technology project aimed at ensuring rural Montour and surrounding counties maintain access to an advanced pharmacy. Funding will be used to purchase and install a new modular compounding suite, upgrade and replace the pharmacy’s HVAC system, add non-sterile hazardous drug compounding capacity, and improve the overall workflow layout of the facility.Central
GSL Hospital dba Geisinger St. Luke's Hospital $         948,927.00Geisinger St. Luke’s Hospital will use this one-time program payment to fully equip a fourth operating room (OR 4) in response to sustained growth in surgical demand. y. Expanding operating room capacity will enable the hospital to perform approximately 1,700 additional surgical procedures annually, serving as a primary benchmark for project success. Northeast
Lehigh Valley Hospital - Pocono $         996,328.00The program payment for Lehigh Valley Hospital-Pocono will be used to modify the behavioral health unit, purchase supplies and equipment that stabilize and enhance rural health access. Equipment includes one GE Volusun Expert 22 BT25 ultrasound machine to support both telehealth and in person Maternal Fetal Medicine specialty care. Nine Teladoc TV Pro 300 carts will allow the hospital to connect bedside patients with remote clinicians and have these visits integrated with the electronic medical
record. Two Retina Vue Scanners will provide retinal screening for more than 3743 patients with diabetes. And there will be an update to install anti-ligature door upgrades to 12 rooms in our behavioral health unit. These upgrades are essential for patient and staff safety.

St. Luke's Hospital - Monroe Campus  $         823,050.00St. Luke’s Hospital – Monroe Campus plans to use this one-time payment to equip a sixth operating room to meet increased patient demand. With high utilization rates in its other ORs, SLMO has finished the OR space and now it needs to be outfitted for use.Northeast 
Wellspan Chambersburg Hospital $         940,153.00WCH is using the funding for the Neuro Navigation and 3D imaging Project, which requires the purchasing of eqipment. Using existing navigation and visualization equipment, WCH neurosurgical team completed 707 cases with an average operative time of 145.4 minutes per case in CY2025. With the addition of enhanced navigation and visualization technology and related tools, we anticipate an immediate increase 35 cases per year with an annual increase of 3.9%, providing access to 193 additional procedures in 3 yearsSouth Central 
WellSpan Evangelical Community Hospital $     1,000,000.00The purchase and installation of ten new automated transfer switches is a one-time project that will stabilize electrical systems, which immediately and automatically connect critical hospital systems to back-up generators in the event of a local or regional power outage. During 2025 these ten units were inspected by a third-party consultant and recommended for replacement due to being obsolete and replacement parts no longer being available.Central

Lehigh Valley Hospital Schuylkill $         997,428.00The program payment for Lehigh Valley Hospital-Schuylkill will be used to purchase supplies and equipment that stabilize and enhance rural health access. These supplies and euipment include an ultrasound machine to support both telehealth and in person Maternal Fetal Medicine
specialty care. Teladoc TV Pro 300 carts that will  allow the hospital to connect bedside patients with remote clinicians and have these visits integrated with the electronic medical  record. Smart beds will replace beds across acute care units to prevent falls and improve patient outcomes. Five Retina Vue Scanners will provide retinal screening for more than 2605 patients with diabetes. Twelve Stryker Medical stretchers with 30” wheels, 700lbs capacity and a scale will allow for seamless transport through the Emergency Department as well assist with accurate vital signs.

Armstrong County Memorial Hospital (ACMH) $         987,284.00The ACMH Infrastructure Enhancement Project is designed to strengthen rural healthcare delivery capacity by modernizing critical infrastructure, improving access to care, and enhancing operational efficiency. The project focuses on upgrading clinical equipment and supporting facility improvements to better serve the community.  This includes the purchase of a mammography unit, a breast cancer ultrasound machine, and a medical vacuum system. Southwest
Bucktail Medical Center$78,895By not having to shut the facility down during warm temperatures, it will maintain access to care in the rural communities of Western Clinton 

Bucktail Medical Center faces a significant and ongoing challenge related to its aging infrastructure, particularly the hospital’s HVAC system. The installation of minisplit HVAC units offers a practical, cost effective interim solution to a critical infrastructure issue at Bucktail Medical Center. This project directly supports patient safety, protects essential medical equipment, ensures access and continuity of care, and aligns with the hospital’s current financial realities while longer term capital solutions are evaluated.

Carbon Schuylkill Community Hospital dba St. Luke's Miner's Memorial Hospital, St. Luke's Hospital-Miners Campus $         909,231.00The one-time payment will support medical imaging technology and infrastructure enhancements by purchasing a new fluoroscopy system, enabling the hospital to continue this service for patients throughout the rural region. The equipment will replace the current system
that is quickly approaching total failure; no manufacturer parts can be ordered for it and the unit cannot be serviced. Frequent fluoroscopy system down times during FY26 delayed patient care, or patients had no care due to the unsurmountable burden to travel long distances to receive this
diagnostic testing. The one-time payment will also be used to purchase technology equipment required in three operating rooms to support perioperative workflow visibility and remote clinical support functions

Clarion Hospital $         751,107.00Rapid Response funds will support a one-time capital investment to replace the aging 17-year-old CT scanner at Clarion Hospital, ensuring the stabilization and continued availability of essential diagnostic imaging services in Clarion County.Northwest
St. Luke's Hospital - Carbon Campus $         996,519.00St. Luke’s Hospital – Carbon Campus plans to use this one-time payment to purchase new equipment and update a shell space for sterile storage for our operating rooms. The equipment is needed either to replace existing equipment that has reached its end of life or to augment existing equipment to meet increased patient demand. The sterile storage room for
operating room instruments and equipment is needed to enhance workflows and efficiency for the operating rooms themselves and the operating room staff.

Barnes-Kasson County Hospital $     1,000,000.00The hospital’s operating room (OR) air handling system has experienced complete failure and is no longer operational. The system has exceeded its useful life, and repair is no longer feasible.
As a result, the operating room cannot be safely used for surgical procedures, required environmental controls cannot be maintained, and infection prevention standards cannot be met. This failure has made the operating room non-functional.  Replacement of the failed operating room air handling system is an urgent, one-time infrastructure investment necessary to stabilize a distressed Critical Access Hospital and preserve access to essential rural health services.

Counselling Services of Southeastern Erie County dba Corry Counseling of LECOM Health $         548,200.00A one-time infrastructure investment to stabilize the Corry Counseling of LECOM Health facility and expand behavioral health service capacity for rural populations. The funding will support an AI-suppprted documentation sustem to improve efficency, video conferencing equipment, repair of water damage and sewage systems, and repurposing of office spece to support clinical capacity. Northwest
Jeffco Health Services, Inc. $         907,296.00This project will replce five air handlers, walk-in refrigeratir and freezer, three elevators, and the basement chiller system. Without these infrastructure upgrades, the ability of PHJM to provide necessary, high-quality care
and a proper home for the elderly population of the area would be jeopardized, greatly increasing the chances of negative health outcomes for the community.
North Central
Robert Packer Hospital $     1,000,000.00The upgrade of the cogeneration engine at Robert Packer Hospital (RPH)  is a critical infrastructure investment necessary to ensure uninterrupted access to essential healthcare services across a rural, multi-campus
system. The existing cogeneration unit is quickly approaching the manufacturerrecommended threshold for major overhaul, at approximately 80,000 run-time hours, which is anticipated to be reached by April 2027.
Northern Tier
Wayne Memorial Community Health Centers $         997,423.00 WMCHC has two minor renovations including in this funding. One of which will increase patient access particularly women’s health and obstetrics in an area (Pike County) where Wayne Memorial Community Health Centers provides the only OB care with the nearest other resources can be an hour away from patients’ homes. The second renovation is adding a gutter system to one office in Wayne County. Heavy rains are deteriorating the pavement and walkways creating a continuous maintenance issue in order provide safe footing entering and exiting. 

The remainder of the budget is comprised of technology embracing AI and innovation.

Wellspan Waynesboro Hospital  $         881,989.00WWH will modify an outpatient clinical space in order to expand access to care and ensure the long-term sustainability of essential rural health services. A key component of the project is the modification of the existing wound clinic area to create a shared, modernized space
with IV therapy. The funding will also replace an ultrasound unit and and echocaridgram unit. 
South Central 
Windber Hospital, Inc. dba Chan Soon-Shiong Medical Center at Windber  $         985,100.00Chan Soon-Shiong Medical Center at Windber will use the Rapid Response Stabilization Program payment to complete critical elevator repairs and modernization that directly support patient safety, access to care, emergency readiness, and uninterrupted hospital
operations. The hospital’s elevators are essential for safely moving patients, staff, medications, supplies, medical equipment, and emergency response personnel between clinical, diagnostic, inpatient, outpatient, and support areas.
Southern Alleghenies
Punxsutawney Area Hospital, Inc. $         605,721.00Funding will be used to repair and reroute a collapsed sewer line, ensure propoer sanitation and hygiene for staff, and repair drainage structure. In addition funding will be used to omplement an IP-based paging, alerting, and visual communication system and replace the wireless handset communications system.  North Central
Meadville Medical Center dba Meadville Medical Center Health Services (MMCHS) $         997,900.00Funds will support the full replacement of rooftop Air Handling Unit (AHU-18) and associated heating, ventilation, structural, electrical, and controls systems serving the majority portion of the 2nd, 3rd, and 4th floors. The existing unit has degraded performance and requires replacement to maintain reliable hospital operations, environmental control, patient comfort, and continuity of care in a rural healthcare setting. Northwest
Corry Memorial Hospital dba LECOM Health Corry Memorial Hospital $     1,000,000.00Corry Memorial Hospital will use funding to upgrade critical technology and infrastructure supporting both Radiology services and Inpatient Critical Care. This includes a new radiology system, ventilator, cardiac monitoring system, call bell system, and Baxter IV pumps. Northwest 
Transitions Healthcare Allens Cove, LLC $         198,524.00Transitions Healthcare Allens Cove will implement a modern Voice over IP (VoIP) telephone system to address critical gaps in communication infrastructure within the facility. The implementation of a VoIP system will enable direct call routing to resident rooms, departments, and staff, significantly improving communication efficiency. Families will be able to connect directly with residents, enhancing accessibility and improving the overall resident and family experience. South Central
Nulton Diagnostic and Treatment Center, P.C dba NDTC $         499,450.00Nulton Diagnostic and Treatment Center, P.C. (NDTC) will implement a centralized technology platform designed to support Rapid Access Psychiatric Services and Community-Based Intensive Treatment (CBIT) across rural Pennsylvania, with initial deployment at its
licensed location in Cambria County. The platform will enable real-time referral intake, scheduling, patient tracking, and performance
monitoring, and consists of two integrated components: (1) a Rapid Access coordination module that allows referring providers, including emergency departments, crisis response systems, and inpatient psychiatric units, to schedule psychiatric appointments within one business day, and (2)
a CBIT management and oversight module that supports providers and managed care organizations in monitoring adherence to the CBIT model and tracking utilization and performance.

Cornerstone Care, Inc. Mt Morris $         449,686.00Cornerstone Care is using funds to expand its capacity to offer pharmacy services at its Community Health Center of Mount Morris site. Cornerstone Care is using new strategies to provide prescription access through a secure vending machine.  This innovative equipment allows the health center to fill prescriptions and make them available for our patients during extended hours.  The project budget also includes new roofing, a generator, IT equipment upgrades, ADA enhancements, pharmacy upgrades, and replacement of outdated dental and medical equipment. The Mount Morris site serves nearly 2,500 unique patients during more than 5,300 visits annually across service lines. Southwest
Cornerstone Care, Inc. Rogersville $         384,840.00Cornerstone Care is using funding for facilities enhancements for the Rogersville Health Center site. Cornerstone Care is requesting funds for a new roof, flooring, generator, IT equipment upgrades, ADA enhancements, and medical equipment. Each requested item will support the maintenance of services provided to the rural community served at this location, which currently serves 1,235 patients and provides over 3,300 visits annually, across service lines.  Southwest
Somerset Care Inc dba Meadow View Nursing Center $         803,539.00The project includes the following components:
• Replacement of outdated and insufficient ceiling lighting throughout resident rooms, hallways, and common areas
• Installation of new, safe, and durable flooring to eliminate current trip hazards and improve mobility and safety
• Interior painting of resident rooms, hallways, and common areas to improve the care environment and resident experience
• Replacement of aging laundry equipment to improve infection control and
operational efficiency
• Installation of access control systems at facility entry points and staff areas to enhance resident safety and security
• Replacement of resident Transportation vehicles for the facility to be able to transport resident to appointments outside of facility in a rural area where other providers are not available to transport
Southern Alleghenies
LaFayette Manor Inc. $         999,995.00Funding will go to urgent infrastrucure improvements and equipment upgrades. One of the most urgent needs is the repaving and replacement of our parking lot asphalt. In addition to exterior access improvements, critical infrastructure upgrades are needed within the facility’s mechanical systems. The replacement of the deaerator tank in our boiler system is essential to maintaining reliable heat and hot water—both vital to resident health, hygiene, and comfort.

Several concrete surfaces across the campus also require repair, including the loading dock, resident courtyard, and both the front and rear sidewalks. These areas have developed cracks and uneven sections that pose serious trip and fall risks, particularly for residents using mobility devices.

The addition of a new accessible bus and van will ensure safe, dependable transport for residents, including those who use wheelchairs or require additional support.

Clarion Psychiatric Center $         998,287.00The requested funding will support a coordinated set of high-impact projects that collectively improve safety, functionality, and long-term sustainability.
A primary focus of the proposed funding is the modernization of patient care
environments to meet current behavioral health safety and infection control standards.

Upgrades to inpatient unit bathroom fixtures, including showers, toilets, and sinks across 32 patient rooms, will replace deteriorated infrastructure that currently requires frequent repair and presents ongoing infection control and safety challenges. These improvements are expected to reduce maintenance-related service calls in these areas by an estimated 30–40% and significantly decrease the likelihood of unplanned outages that could impact patient room availability. Additional safety enhancements include the replacement of standard shower curtains with behavioral health–appropriate soft magnetic doors designed to reduce ligature risk. This project directly aligns with current behavioral health design standards and is expected to eliminate a known environmental risk factor within 100% of affected
patient bathing areas. The upgrade supports a safer care environment while reducing staff monitoring burden and enhancing patient dignity.

Cornerstone Care, Inc. dba Cornerstone Care Greensboro Health Center $         951,998.00A new dental operatory is planned for the Greensboro site to meet the growing demand for accessible oral health services. A renovation project will be implemented to create a new floor plan for health center efficiency and add a dental operatory, including a new chair and supporting dental equipment, to provide more oral health care for Greensboro adults and children. An additional operatory will increase access and efficiency, allowing the practice to serve more dental patients. 

The project budget also includes a generator, IT equipment upgrades, ADA enhancements, pharmacy, dental, and medical equipment & supplies.

St. Luke's Hospital of Bethlehem, Pennsylvania dba St. Luke's Hospital - Lehighton Campus, formerly Blue Mountain Hospital  $     1,000,000.00St. Luke’s Hospital - Lehighton Campusis seeking a one-time payment to support the replacement of the roof at the Campus, a rural mental health hospital providing essential inpatient and outpatient behavioral health and medical services. This investment will ensure the Campus’ continued operation that provides inpatient psychiatric care for approximately 1,000 patients annually, delivers tens of thousands of outpatient medical and behavioral health visits, and serves as a lifeline for adults, older adults, and veterans at elevated risk for mental health crises.The investment will help safeguard the physical infrastructure necessary to maintain uninterrupted access to these essential services.Northeast
DLP Conemaugh Memorial Medical Center LLC dba Conemaugh Memorial Medical Center $         999,845.00Conemaugh Memorial Medical Center will replace its failing 1996 telecommunications system. The system serves the main hospital and rural clinics across Cambria and Somerset counties. Baseline data includes a 20% call failure rate, up to four daily system restarts and a recent major outage affecting 3,000 extensions. Benchmarks for success will be measured by broader patient outcomes, including achieving a 0% system-driven call failure rate, eliminating dropped calls for rural tele-consults, and ensuring reliable patient access for scheduling and care coordination. Southern Alleghenies
Weatherwood Rehabilitation and Healthcare LLC dba Forrest Hills Rehabilitation & Healthcare Center $     1,000,000.00This program payment will fund a renovation and structural improvement project at Weatherwood Rehabilitation and Healthcare Center, a 200-bed skilled nursing facility in Carbon County, Pennsylvania. Center for Behavioral Health-PA, LLC dba Cranberry Township Comprehensive Treatment Center $         415,232.00Center for Behavioral Health-, LLC DBA Cranberry Township CTC will receive funding (1) to purchase and implement a passenger transportation van that will expand access to medication-assisted treatment (MAT), counseling, and behavioral health services for individuals living in rural and underserved Pennsylvania communities, and (2) for facility enhancement
funding to support critical security infrastructure improvements focused on patient safety, staff protection, operational oversight, regulatory compliance, and long-term operational sustainability. The proposed project includes installation of additional surveillance cameras within the dispensary area, expanded exterior camera coverage upgrades, and installation of surveillance coverage within administrative areas that were not included in the facility’s prior security system upgrade.

Wyoming County Healthcare Center II dba Wyoming County Healthcare Center $         879,750.00Funding will support critical electrical infrastructure upgrades that are essential to the continued operation of healthcare services in a rural county without a hospital. WCHC is an electric-dependent facility housing 14
healthcare-related organizations that collectively supported approximately 46,000 patient visits in 2025.

The project will replace major electrical distribution infrastructure serving the facility, including the main electrical switchboard “MDS,” distribution switchboards and motor control centers, and selected panelboards and transformers. These systems support the continued operation of healthcare providers, diagnostic services, emergency response functions, telecommunications and data systems, life-safety
systems, elevators, environmental controls, and other building systems required to maintain safe and reliable access to care.
Northern Tier
Board of Directors of the Rouse Estate dba Warren County Rouse Home $         370,886.00The Rouse Home will use funding to purchase 120 specialized low-profile beds designed to improve resident safety, clinical outcomes, and dignity for skilled nursing residents. These beds are equipped with advanced technology that allows the bed frame to lower completely to floor level, significantly reducing the risk of falls and injuries associated with bed-related incidents. Research and manufacturer data demonstrate these beds can reduce falls by approximately 55% and reduce bed fall injuries by up to 100%.Northwest
CMSU Behavioral Health and Developmental Services $         166,443.00CMSU will use the funds to replace aging Information Technology equipment that is at or nearing its end of life. These devices are used by CMSU staff who are in the field, meeting residents in their home eliminating the transportation barriers that many residents experience. These mobile devices afford CMSU staff connection to clients and allow clients to connect with other needed services. As these devices age, the ability to operate effectively and securely is compromised, creating undue stress on an already vulnerable workforce. These devices also allow staff to remain virtually connected to their home office, supervisor support, and training. 

CMSU will also purchase a passenger van to replace an aging model that has accrued high mileage. The current van is less reliable, decreasing quality and continuity of care. 
Central
Mainstream Counseling $         452,800.00The project addresses substantial physical barriers that currently limit equitable access to behavioral health services at 905 Washington Street.

Although portions of the building were renovated to support psychiatric and MAT treatment services, the facility currently lacks a fully ADA-compliant accessible route from designated parking areas into the treatment environment. Individuals with mobility limitations, chronic medical conditions, or physical disabilities face unnecessary barriers to independently accessing behavioral health treatment services.

Program funding will support construction of an ADA-accessible route from designated handicapped parking spaces to the treatment facility through installation of a compliant exterior ramp system leading to an elevated deck area and direct building entry.

The project also addresses a significant interior life-safety and patient access issue affecting second-floor clinical space currently utilized for behavioral health treatment.

The existing interior stair system limits safe and efficient access to second-floor treatment areas due to inadequate pitch and spatial limitations. This creates challenges for patients, staff, and individuals with mobility concerns seeking behavioral health services upstairs.

The project also addresses infrastructure conditions that threaten the long-term viability of behavioral health treatment space at the 905 Washington Street facility.

Roof deterioration and water intrusion have affected portions of the building, including areas supporting existing behavioral health services. Without intervention, continued deterioration risks progressive damage to treatment space, increased maintenance costs, and potential reduction in usable licensed service capacity.

PA’s RHTP is informed by extensive stakeholder input over the past two years, including Governor’s roundtables, listening sessions, regional rural health summits, and more than 300 public responses and comments. The plan establishes a rural health framework that emphasizes access, outcomes, workforce development, technology, innovation, and sustainability. The plan focuses on investments that create lasting infrastructure, strategic partnerships, and build the healthcare workforce.

I am the low hum of wind through the tall, green corn,
The deep, earthy scent of hay freshly cut, a memory vibrant.
I am the dirt in these boots, ingrained, deep.
From sun-up chores to starlit quiet.

I grew up where the horizon stretched, uninterrupted,
A tapestry woven by seasons, hiding nothing.
Barefoot on welcoming porch swings, chasing fireflies as dusk settled,
Learning life's vital lessons, from dawn's first glimpse ‘til sunset’s fading light.

I pledge my hands, ..4H rooted lessons deep within me.
From the nervous flutter of public speaking to
raising market lambs, facing a judge's assessing eye.
Community built, leadership practiced,
Friendships forged, a silent pride swelling, expanding over time.
Planting seeds of future dreams, a hope every rural soul carries.

But shadows lengthen now, across our grand fields,
A quiet, growing struggle, woven into the rural landscape.
For the promise of revitalization, won't truly take hold, won't truly ascend,
Unless we tend to the well-being of our people, with all our collective strength.

Imagine the worry pressed onto faces, when a baby's almost here,
And the nearest maternity care is miles, hours away.
Anxious drives, hurried breaths, gnawing isolation.
Our mothers, our infants, deserve a gentle, nearby embrace,
Not endless roads, or sterile waiting rooms, a cold forgotten space.

And quiet battles fought alone, behind closed farmhouse doors,
Farmer suicide is real! Burdens carried in silence, until they can be carried no more.
A whispered plea, a fear of shame that endures, keeping people hidden,
While silent struggles fester, and rural sadness grows.
We need counselors, therapists, hands reaching out to find,
A path to healing, to understanding, for body and for mind.

The wisdom of age, a cherished gift, yet in rural lands, it's often a silent strife.
For our elders, the golden years can bring isolation, a lonely, quiet life.
Access to specialists, in-home care, and community's gentle embrace,
Are often scarce resources, leaving vulnerability in their place.
We must ensure our seniors, who built these fields and bore our rural pride,
Have comfort, dignity, and care, for them we must abide.

The open road, a symbol of freedom, becomes a barrier to in rural space,
When miles stretch long for folks to reach a healing, welcoming place.
Public transport is a whisper, oft unheard, unseen,
Leaving many without a ride, to reach their healthcare needs.
Emergency rooms grow distant, a lifeline hard to seize,
And chronic conditions worsen, bringing rural communities to their knees.
Let us think of innovative solutions, transport woven into our design,
So that access to care is not a luxury, but a right, for all to find.

So as we speak of progress, ambitious plans, economic gain,
Remember these deep currents, that shape our current pain.
