Instructions
You must follow these procedures in order for us to process the certification request:
- The request must be made in writing.
- The request must be accompanied by:
- The name of the company
- The 5-digit NAIC company code
- The documents to be certified OR a clear description of documents we have on file that you wish to be certified
- The number of certificates and copies of certificates required
- The name and address of the person to receive the certificates
- Provide copies of the documents to be certified. We will certify documents currently on file, and there is a per-page charge for copies.
- Submit payment in advance. Please provide a name and telephone number, allowing us to contact the individual with the payment amount.
The cost for certifications are $20 per certificate prepared and $0.20 per page certified.
Example 1
Certification of a two-page document provided by the requester would cost:

The cost of copying documents currently on file with the Department are:
- $2.00 per legal-size page
- $0.25 per letter-size page
There is no per-page certification fee for documents we have on file.
Example 2
Certification of a four-page letter-size document and a separate 1-page legal-size document on file with the Department would cost:

Note: Any report of examination cannot be certified until 30 days after the order is signed.
