ORDER SUMMARY:
1. Robert DeMott (“DeMott”), as a Constable/Deputy Constable in the Commonwealth of Pennsylvania, failed to timely file a Statement of Financial Interests for the 2024 calendar year in compliance with Section 1104(a) of the Public Official and Employee Ethics Act, 65 Pa.C.S § 1104(a).
2. Per the Consent Agreement of the parties, we hereby levy a civil penalty against DeMott in the total amount of $100.00 for DeMott’s delinquent Statement of Financial Interests for calendar year 2024.
3. To the extent DeMott has not already done so, DeMott is ordered to make payment of the aforesaid civil penalty in the total amount of $100.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing this Commission’s electronic payment system or by forwarding to this Commission a certified check or money order in the amount of $100.00 made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
4. Failure to comply with Paragraph 3 of this Order will result in the initiation of an appropriate enforcement action.

ORDER SUMMARY:
1. Ann O’Haire (“O’Haire”), as a Human Services Licensing Representative with the Pennsylvania Department Human Services, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act, 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2023 with the Pennsylvania Department of Human Services.
2. This Commission hereby levies one maximum civil penalty against O’Haire at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2023 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00). O’Haire is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission’s electronic payment system or by forwarding to the Pennsylvania State Ethics Commission a check made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
3. O’Haire is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2023 with the Pennsylvania Department of Human Services by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.
4. Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

ORDER SUMMARY:
1. Shakena Tate (“Tate”), as a Tax Examiner 2 with the Pennsylvania Department of Revenue, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act, 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2023 with the Pennsylvania Department of Revenue.
2. This Commission hereby levies one maximum civil penalty against Tate at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2023 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00). Tate is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission’s electronic payment system or by forwarding to the Pennsylvania State Ethics Commission a check made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
3. ORDER SUMMARY:
1. Dawn Durst (“Durst”), as a Materials Supervisor with the Pennsylvania Department of Transportation, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act, 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2023 with the Pennsylvania Department of Transportation.
2. This Commission hereby levies one maximum civil penalty against Durst at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2023 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00). Durst is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission’s electronic payment system or by forwarding to the Pennsylvania State Ethics Commission a check made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

1. 2. This Commission hereby levies a maximum civil penalty against Almodovar at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2023 has remained delinquent, for a total civil penalty of Two Hundred Fifty Dollars ($250.00). 2. 2. This Commission hereby levies one maximum civil penalty against Genzlinger at the rate of Twenty-Five Dollars ($25.00) per day for each day the Statement of Financial Interests for calendar year 2024 remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).
