ORDER SUMMARY:
1. Shelley Dixon (“Dixon”), as an Early Learning Program Certification Representative with the Pennsylvania Department of Human Services (“Human Services”), failed to timely file a Statement of Financial Interests for the 2023 calendar year in compliance with Section 1104(a) of the Public Official and Employee Ethics Act, 65 Pa.C.S § 1104(a).
2. Per the Consent Agreement of the parties, we hereby levy a civil penalty against Dixon in the total amount of $150.00 for Dixon’s delinquent Statement of Financial Interests for calendar year 2023.
3. To the extent Dixon has not already done so, Dixon is ordered to make payment of the aforesaid civil penalty in the total amount of $150.00 by no later than three months after the mailing date of this Order, by utilizing this Commission’s electronic payment system or by forwarding to this Commission a certified check or money order in the amount of $150.00 made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
4. To the extent Dixon has not already done so, Dixon is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2023 with Human Services by no later than the thirtieth (30th) day after the mailing date of this Order, with a copy of such filing forwarded to this Commission.
5. Failure to comply with Paragraph 3 of this Order will result in the initiation of an appropriate enforcement action.

Economic Development Consultant 2

ORDER SUMMARY:
1. Michael Chapaloney (“Chapaloney”), as Economic Development Consultant 2 with the Department of Community and Economic Development, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act, 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2023 with the Department of Community and Economic Development.
2. This Commission hereby levies one maximum civil penalty against Chapaloney at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2023 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00). Chapaloney is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission’s electronic payment system or by forwarding to the Pennsylvania State Ethics Commission a check made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

Chapaloney is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2023 with the Department of Community and Economic Development by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.

ORDER SUMMARY:
1. Amber Lutz (“Lutz”), as Clerk 3 with the Department of Labor and Industry, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act, 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2022 with the Department of Labor and Industry.
2. This Commission hereby levies one maximum civil penalty against Lutz at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2022 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00). Lutz is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission’s electronic payment system or by forwarding to the Pennsylvania State Ethics Commission a check made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

ORDER SUMMARY:
1. Megan Thompson (“Thompson”), as a Tax Account Collections Technician with the Department of Revenue, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act, 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2023 with the Department of Revenue.
2. This Commission hereby levies one maximum civil penalty against Thompson at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2023 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00). Thompson is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission’s electronic payment system or by forwarding to the Pennsylvania State Ethics Commission a check made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

Thomas Keys (“Keys”), as Senior Applications Developer with the Executive Offices, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act, 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2022 with the Executive Offices.
2. This Commission hereby levies one maximum civil penalty against Keys at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2022 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00). Keys is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission’s electronic payment system or by forwarding to the Pennsylvania State Ethics Commission a check made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
