The Pennsylvania State Ethics Commission has issued a final adjudication in the following matters under

HARRISBURG, PA –

Order No. 774-S (Demchuk)
Special Investigator 2
Department of Revenue
Bucks County, PA

ORDER SUMMARY:

1.         William Demchuk ("Demchuk"), as a Special Investigator 2 with the Pennsylvania Department of Revenue ("Department of Revenue"), failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act ("Ethics Act"), 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2021 with the Department of Revenue.

2.         This Commission hereby levies one maximum civil penalty against Demchuk at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2021 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Demchuk is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3.         Demchuk is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2021 with the Department of Revenue by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.

4.         Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

 

Order No. 775-S (Stone)
Income Maintenance Caseworker
Department of Human Services
Philadelphia County, PA

ORDER SUMMARY:
 1.         Marsheeta Stone ("Stone"), as an Income Maintenance Caseworker with the Pennsylvania Department of Human Services ("Department of Human Services"), failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act ("Ethics Act"), 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2021 with the Department of Human Services.

2.         This Commission hereby levies one maximum civil penalty against Stone at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2021 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Stone is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

Income Maintenance Caseworker
Department of Human Services
Luzerne County, PA
 

1.         Gloridy Leon ("Leon"), as an Income Maintenance Caseworker with the Pennsylvania Department of Human Services ("Department of Human Services"), failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act ("Ethics Act"), 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2021 with the Department of Human Services.

2.         This Commission hereby levies one maximum civil penalty against Leon at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2021 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Leon is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

Income Maintenance Caseworker

1.         Revathy Kithcart ("Kithcart"), as an Income Maintenance Caseworker with the Pennsylvania Department of Human Services ("Department of Human Services"), failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act ("Ethics Act"), 65 Pa.C.S. § 1104(a), when Kithcart failed to file a Statement of Financial Interests for calendar year 2021 with the Department of Human Services.

2.         This Commission hereby levies one maximum civil penalty against Kithcart at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2021 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Kithcart is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3.         Kithcart is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2021 with the Department of Human Services by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.

Corrections Officer 4
Department of Corrections
Somerset County, PA
 

1.         Carl Thomas ("Thomas"), as a Corrections Officer 4 with the Pennsylvania Department of Corrections ("Department of Corrections"), failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act ("Ethics Act"), 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2021 with the Department of Corrections.

2.         This Commission hereby levies one maximum civil penalty against Thomas at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2021 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Thomas is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3.         Thomas is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2021 with the Department of Corrections by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.

4.         Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.
