Submit a DEP Air Pollution Control Act Compliance Review Form

How to apply

  1. Submit the form to the right Bureau of Air Quality office or local agency. If ownership changes, include the new owner's info, the compliance review form, and the necessary permit change forms and fees.

  2. Provide information for the past 5 years.

  3. For corporations, use the name from the Articles of Incorporation in Sections A and B. For others, provide the legal name(s) and documentation.

  4. A corporate officer or a qualified employee must sign for corporations. If needed, attach the corporate seal. If not required, just state that.

  5. For large submissions, follow the form's outline. Use tabs for sections, number pages, and include a table of contents.

  6. Answer all questions. Use "None" or "Not Applicable" where appropriate. Extra information might be needed later.

  7. Incomplete forms will be rejected and returned, delaying your application.
